I know this has been brought up before, but I wanted to know when you all usually write up an accident report (home daycares). I have a little boy who bumped his head a train table handle and he has a lump and a small cut. Should I write a report to the parents or should I reserve these for more serious injuries?

I have an "Owie Report" form that goes in every child's file. It's says "Billy's Owie Report" on the top and underneath are just two headings. One says "When and Where" and the other says "What Happened"
I just write down for example "Monday 12th March 2012 - driveway" under the first column and "Fell and skinned left knee" under the other. The parents initial at the end of the day.
More serious injuries are to be reported on a form the state supplies.
I send home the state report on scrapes.
I prefer to cover my hiney.

I'd fill out an incident report.
My report is pretty generic but it's:
Who: Who got injured
What: On what or doing what
When: Time it happened
Where: Inside, outside, etc
How: How it happened. Bobby was playing with the train set. He leaned over and bumped his head on the table resulting in a bruise with a small cut.
Then I list what I did: The area was cleaned and used a cold, wet rag.

Yup, what everyone else said! Definitely have parents initial and both parties get a copy.

If it leaves a mark always write it up.
Always let them know if it's a head bump so they can monitor him/her at home as well.
